Immunization Requirements

It is the policy of the University Health Center to comply with the mandatory immunization program established by the Board of Regents of the University System of Georgia. These requirements include documented proof of immunity to measles, mumps, rubella, varicella (chicken pox), tetanus, and hepatitis B, and completed screening questionnaire for tuberculosis prior to registration at the University of Georgia. All entering students must meet these requirements, and no clearances are given to allow registration without meeting these requirements. For the required forms, please see Immunizations.

Print Your Immunization Records

If you are currently enrolled or a recent graduate (within the last 3 months) you may print a copy of your immunizations by accessing the UHC Patient Portal:

  1. Log in
  2. Select “Immunization” from left side of screen
  3. Select “Print” from top right corner

Transferring Health Information to Another School or Doctor

The Registration and Health Information department will be happy to assist you with transferring your health information and/or immunization records to another academic institution, physician or medical facility. You must make your request in writing. Be sure to include your nine digit UGA ID number (810———) number and your signature. If you are requesting PHYSICAL HEALTH information only, you can fill out the Release of Health Information form using the link on this page. Bring it to the Registration and Health Information department or FAX it to 706-542-4959. Use the Release of Mental Health Information form if you want to release MENTAL HEALTH records from Counseling and Psychiatric Services (CAPS). Mail your request to Counseling and Psychiatric Services or FAX it to 706-542-8661. You may also make a request in person at the patient registration desk at the health center. It usually takes at least 24 hours to process. There is no charge for this service if it is going to another health care facility, doctor’s office or school.

Requesting Health Information

If you request a copy of your health information to be given directly to you,
there will be a charge of $.25 per page after the first four pages. For
an insurance company or legal office, there is a $25.00 retrieval, copy and processing.

Medical Authorization

If you choose to authorize another person or persons to have access to your health records, you will need to fill out, sign and submit the Medical Authorization to Obtain Health Information form for PHYSICAL HEALTH information only. You can send the form to UHC or FAX it to 706-542-4959. Use the Release of Mental Health Information form to allow someone to have access to your MENTAL HEALTH records through Counseling and Psychiatric Services (CAPS). You can send the form to CAPS or FAX it to 706-542-8661.
