Health Information and Immunization Records


Transferring Health Information

The Patient Registration and Health Information Department will be happy to assist you with transferring your health information and/or immunization records to another academic institution, physician or medical facility. You must make your request in writing. Be sure to include your SSN number and your signature. You can also fill out the Release of Health Information form. Fax or mail your request to the Patient Registration and Health Information Department. You may also make a request in person at the patient registration desk at the health center. It usually takes at least 24 hours to process. There is no charge for this service if it is going to another health care facility, doctor's office or school.

Immunization Requirements

It is the policy of the University Health Center to comply with the mandatory immunization program established by the Board of Regents and the University System. These requirements include documented proof of immunity to measles, mumps, and rubella, varicella (chicken pox), tetanus, and hepatitis B, prior to registration at any college or university within the University System. No temporary clearances are given to allow registration without meeting these requirements. For more information on Immunization requirements, please see Immunizations.

Requesting Health Information

If you request a copy of your health information to be given directly to you, there will be a charge of $.25 per page after the first four pages. For an insurance company or legal office, there is a $25 charge per volume.
